Degloving of the Forearm with Reconstruction Using Acellular Dermal Matrix

Traumatic complex wounds of the upper limbs exhibit significant prevalence in emergency services. Traffic accidents are among the leading etiologies of these injuries, which can extend from the skin to deep structures, causing severe functional and aesthetic impact. These wounds offer treatment options through reparative surgical procedures in Plastic Surgery, along with complementary options for the complete healing process. Acellular Dermal Matrix (ADM), for instance, constitutes an option for covering complex forearm wounds. The aim of this work is to report the use of ADM in the definitive treatment of a complex wound resulting from traumatic degloving of the forearm. This concerns the case of a 15-year-old male patient with an extensive injury to the posterior aspect of the right forearm, involving loss of substance and tendon and muscle exposure, following a car accident. Treatment involved clinical support and surgical procedures for graft recipient site preparation. Thus, ADM was utilized, followed by partial skin autografting with successful integration. In conclusion, this study demonstrates the successful use of ADM in a patient with a complex forearm wound, facilitating healing and recovery of anatomical structures, with excellent aesthetic and functional outcomes.
